What is a medical fitness certificate?

A medical fitness certificate is a statement by a registered medical practitioner that they examined a person on a given date and found them fit (or not fit) for a stated purpose, such as joining a job, admission to college or taking part in a sport. Employers and institutions ask for it before joining.

This format is for doctors and clinics. Only a registered medical practitioner who has examined the person can issue a medical fitness certificate. If you need one, take this format to your doctor; never fill in a certificate in a doctor's name.

How to fill in a medical fitness certificate

  1. Add the clinic letterhead: doctor's name and qualification, registration number, clinic address and phone.
  2. Enter the person's name, age and an identification mark.
  3. Enter the date of examination and what the person is being certified fit for.
  4. Write the examination notes, such as height, weight, blood pressure, pulse and vision.
  5. Choose the result: fit, fit with restrictions, or not fit at present.
  6. Sign with the registration number and the clinic seal; the person examined signs too.
